What is ABDM?
The Ayushman Bharat Digital Mission (ABDM) is India's national initiative to build a unified digital health infrastructure. Launched by the Ministry of Health and Family Welfare and managed by the National Health Authority (NHA), ABDM aims to digitise every Indian's health record and make it accessible — with consent — across the country's healthcare ecosystem.
For healthcare providers and health-tech companies, ABDM integration means becoming part of this national ecosystem: sharing health records, enabling patient-controlled access, and connecting with India's digital health stack.
Who Needs to Integrate with ABDM?
- Hospitals and Multi-Specialty Clinics
- EMR (Electronic Medical Record) and HMS (Hospital Management System) vendors
- Diagnostic labs and LIS (Laboratory Information Systems)
- Telemedicine platforms
- Pharmacy management software
- Radiology / Imaging (RIS) platforms
- Health insurance companies (for HIU access)
- Wellness and chronic care apps
The 3 Milestones of ABDM Integration
Milestone 1: ABHA Creation and Verification
This is the entry point for ABDM integration. In Milestone 1, your platform integrates APIs to:
- Create ABHA numbers for patients (via Aadhaar or Driving Licence)
- Verify and link existing ABHA numbers to patient profiles
- Scan ABHA QR codes at registration desks
- Display ABHA card within your application
Milestone 1 is typically completed in 2–4 weeks and is the minimum requirement for ABDM participation.
Milestone 2: HIP (Health Information Provider) Integration
In Milestone 2, your platform becomes a Health Information Provider — meaning you can share patient health records (prescriptions, lab reports, discharge summaries) through the ABDM ecosystem. This involves:
- Linking care contexts (visits/encounters) to ABHA numbers
- Receiving and responding to health record discovery requests from the ABDM gateway
- Sending FHIR R4-compliant health record bundles to patients' ABHA-linked health lockers
- Managing patient consent flows
Milestone 2 typically takes 4–8 weeks depending on your existing system's maturity.
Milestone 3: HIU (Health Information User) Integration
In Milestone 3, your platform can also consume health records from the ABDM ecosystem — not just share them. A Health Information User can:
- Request patient consent to access health records from other providers
- Receive and render FHIR health record bundles
- Provide care teams with a complete longitudinal view of patient history
Milestone 3 adds another 4–6 weeks and enables the full power of ABDM for clinical decision support.
Technical Requirements for ABDM Integration
FHIR R4 Compliance
All health records shared via ABDM must follow the HL7 FHIR R4 standard. This means your platform needs to generate structured FHIR resources — Patient, Condition, Observation, MedicationRequest, DiagnosticReport, ImagingStudy, etc. — packaged into FHIR Bundles.
NHA Gateway Connectivity
Your platform must register on the NHA gateway and complete sandbox testing before going live. This involves setting up a Bridge URL (webhook) to receive gateway callbacks and implementing ABDM's callback APIs.
Encryption and Security
ABDM mandates end-to-end encryption for all health data. Shared records must be encrypted using the consent artefact's encryption key before transmission.
ABDM Sandbox and NHA Certification
Before going live, every ABDM integration must pass NHA's sandbox testing. This involves setting up test ABHA numbers, simulating consent flows, validating FHIR bundles, and getting NHA's approval for production access. Medi4u provides complete sandbox setup and NHA certification support.
How Long Does ABDM Integration Take?
| Milestone | Timeline |
|---|---|
| Milestone 1: ABHA Creation | 2–4 weeks |
| Milestone 2: HIP Integration | 4–8 weeks |
| Milestone 3: HIU Integration | 4–6 weeks |
| Full Integration | 8–16 weeks |
Conclusion
ABDM integration is no longer optional for serious healthcare platforms. As NHA enforces participation requirements and patients demand digital health access, integrating ABDM gives your platform a competitive edge, compliance credibility, and access to India's national health infrastructure.
